With all the security measures in place with personal computers and mail servers we have experienced a few issues with completed online forms being received. Though the incidents were few, we feel it necessary to make sure the process is as fair as possible. Therefore we have chosen to provide a link to the Scholarship Application in a Microsoft Word format. Click on the link and choose to save. After completion of the Scholarship Application  please send it as an attachment via e-mail to scholarship@lshealthcare.org . 

All required attachments must be included with the Application at the time of submission.  Please contact scholarship@lshealthcare.org via e-mail if you have any questions concerning this procedure.

Applications may also be sent in hard copy, along with required attachments, to the P.O. Box listed below.
