With all the security measures in place with personal computers and mail servers we have experienced a few issues with completed online forms being received. Though the incidents were few, we feel it necessary to make sure the process is as fair as possible. Therefore we have chosen to provide links to the Grant Reports in a Microsoft Word format. The first link is to the Interim Grant Report that should be filed during the project as a progress report. The second link is the Final Grant Report. This report should be filled out and submitted after the project has been completed. Click on the required link and choose save. After completion of either Grant Report  please send it as an attachment via e-mail to grant@lshealthcare.org . 

All required attachments must be included with the Report at the time of submission.  Please contact grant@lshealthcare.org via e-mail if you have any questions concerning this procedure.

Reports may also be sent in hard copy, along with required attachments, to the P.O. Box listed below.
